We may ask you to reschedule if you arrive for your appointment more than 10 minutes after your scheduled appointment time. Please try to arrive 10 to 15 minutes early for your appointment to complete any paperwork associated with your visit.

Please call the office prior to your appointment to cancel. 24 hour notice is greatly appreciated. If you don’t show up for your appointment twice, you will be charged $27, for which you are responsible since insurance will not cover this fee.
